In this document, we address the following concerns and questions:
  1. What personally identifiable data is collected?
  2. Who is collecting my personally identifiable data?
  3. For what purposes is the information collected used?
  4. With whom is the information shared? To whom could the information be sold?

We at MyHealthReach understand how much you value your privacy concerning your health. Our goal is to provide tools that are informative, helpful, and fun to use, while not compromising your right to privacy. We hope that this Privacy Policy for the MyHealthReach Workbook sets your mind at ease regarding our commitment to your privacy. NOTE that by using the MyHealthReach Workbook site, you agree to the terms of this Privacy Policy.

As with the internet in general when using certain sites it is important to note that any information (whether personal or non-personal) that you share in a MyHealthReach Workbook bulletin board, chat room or event, or mail is not governed or protected by this Privacy Policy. Other users of the MyHealthReach Workbook will see your submissions. If you link to a third party site from the MyHealthReach Workbook, any information you reveal there is also not subject to the policies set forth here. 2. Collection of Information

Registration and Profile
We use the personal information that you provide in your MyHealthReach Workbook registration to help you maintain a healthy lifestyle. Your health is our prime concern. We use your personal information to assist in your goal of a healthy lifestyle.

You have the option of not including any personally identifiable information in your registration. MyHealthReach nurses review the information to assess your current condition and help to identify any health risks, which may be present. THIS IS NOT A SUBSTITUTE FOR SEEING YOUR DOCTOR REGULARLY. However, you do have the option of keeping your personal information from the MyHealthReach nurses. This option can be found under the profile tab. Please note that this option only pertains to any additional information that you may add to the MyHealthReach website. The MyHealthReach nurses will retain access to claims data and other information, which is gathered for the purposes of utilization and case/care management for your health benefit plan. If you choose to allow MyHealthReach nurses to view your confidential information, your personal data will be collected and kept confidential by MyHealthReach Online and is subject to this Privacy Policy. The purpose of this site is to serves as an educational tool that is designed to help you stay healthy!

Interactive Features (My Calendar, My Profile, and My Nutrition)
By definition, for the interactive features on the MyHealthReach Workbook to be useful, you must input personal information. All information that is received, used, or disclosed in connection with interactive features of the website is encrypted to maintain confidentiality. MyHealthReach Workbook collects the information you voluntarily provide to store in the interactive functions. The only entities that will have access to the data from these features are the MyHealthReach Workbook itself and certain third parties hired by MyHealthReach to help us operate, improve, and maintain the website. We require that these third parties hold your information in strict confidence and not disclose such information to anyone and to view such information only in the fulfillment of their job duties with the website. MyHealthReach will not share any of the personally identifiable information that is stored in the interactive functions with any other parties not covered by the privacy policy without your explicit permission.

Message Boards
Message Boards are considered to be public forums. This Privacy Policy does not protect you when you use the MyHealthReach Workbook public forums. As a service to our users, MyHealthReach Workbook hosts message boards so that users can share information and give support to each other. You must be aware that any information shared in a public forum is public information and may be viewed or collected by a third party whose actions are not governed by this Privacy Policy. MyHealthReach nurses or the MyHealthReach Workbook will not monitor such message boards.

Click Stream
The MyHealthReach Workbook compiles data obtained by tracking users’ click streams. A click stream is simply the series of MyHealthReach Online web pages viewed by a particular user. The reason that we collect data on click streams is to track which features of the MyHealthReach Workbook are most popular. In order to continually improve and expand our range of services, we gather information on what features are helpful, and what features should be eliminated. Your click stream has no personally identifiable information and no medical information in it. The use of data obtained by tracking your click stream is governed by this Privacy Policy.

3. Utilization of Information

Disclosure of Information
Except as described in this Privacy Policy or specifically agreed to by you, the MyHealthReach Workbook will not disclose any personal information gathered from you on the website. Cases in which we may release personal information to another party include: a) compliance with legal requirements such as a subpoena, court order, law, government regulation, or search warrant; or b) in the case of a physical threat to you or other people. In accordance with the law, MyHealthReach will not disclose your medical records. MyHealthReach may disclose personal information to its corporate subsidiaries. Any personal information provided to MyHealthReach subsidiaries will be treated according to the terms set forth in this Privacy Policy.

MyHealthReach Workbook operations and maintenance contractors sometimes have limited access to your Personal Information in the course of providing products or services to MyHealthReach Workbook. These contractors include vendors and suppliers that provide us with technology, services, and/or content related to better the operation and maintenance of our website. Unless you have opted out of receiving emails and newsletters from MyHealthReach Online, these contractors also may have access to your email address to send newsletters to you on our behalf or to send emails to you for purposes such as conducting market research on our behalf. Access to your Personal Information by these contractors is limited to the information reasonably necessary in order for the contractor to perform its limited function for the MyHealthReach Workbook. We also contractually require that our operations and maintenance contractors 1) protect the privacy of your Personal Information consistent with this Privacy Policy, and 2) not use or disclose your Personal Information for any purpose other than providing us with products and services.

Certain content and services offered to you through our website are served on websites hosted and operated by a company other than MyHealthReach ("Third Party Contractor Websites"). MyHealthReach Workbook does not disclose your Personal Information to these Third Party Contractor Websites without your consent, but you should be aware that any information you disclose once you access these other sites is not subject to this Privacy Policy. MyHealthReach Workbook does not endorse and is not responsible for the privacy practices of these Third Party Contractor Websites. You should review the privacy policy posted on the other site to understand how that Third Party Contractor Website collects and uses your Personal Information. MyHealthReach makes an effort to make it obvious to you when you leave our website and enter a Third Party Contractor Website, either by requiring you to click on a link or by notifying you on the site. In addition, if you see a phrase such as "Powered by" or "in association with" followed by the name of a company other than MyHealthReach, then you are on a website hosted by a company other than MyHealthReach.

MyHealthReach is a contractor and provides co-branded content and services to websites hosted and operated by companies other than MyHealthReach Workbook ("Channel Partner Websites"). You can only access these co-branded content and services through the Channel Partner Website, and usually from the health section of these other websites. The co-branded MyHealthReach Workbook pages that you may access through a Channel Partner Website may have different registration processes and opportunities for information collection, and Personal Information that you provide on these pages may be shared with the Channel Partners. Each of these co-branded MyHealthReach Workbook sites has its own privacy policy posted on that site, which explains what information is disclosed by MyHealthReach Workbook to the Channel Partner and vice versa. If you visit one of these co-branded MyHealthReach Workbook sites, please read the privacy policy that is posted on that site, as well as the individual privacy policy of the Channel Partner Website.

Disclosure to Linked Sites
In addition to the Third Party Contractor Websites that you may access as described above, for your convenience there are links to websites operated by companies other than MyHealthReach Workbook ("Third Party Websites") that are not contractors who provide content or services through our website. These links may be found in advertisements, referenced within content, or placed beside the names or logos of sponsors. The MyHealthReach Workbook does not disclose your Personal Information to these Third Party Websites and the MyHealthReach Workbook does not endorse and is not responsible for the privacy practices of these sites. If you choose to link to one of these Third party Websites, you should review the privacy policy posted on this other site to understand how that Third Party Website collects and uses your Personal Information.

Disclosure of Aggregate Information
MyHealthReach Workbook may provide to other parties non-personal information about you that does not allow you to be identified or contacted and that is combined with the non-personal information of other users ("Aggregate Information"). For example, we might inform third parties regarding the number of users of our site and the activities they conduct while on our site. We might also inform a pharmaceutical company (that may or may not be an advertiser on our site) that "30% of our users live east of the Mississippi" or that "25% of our users have tried alternative medicine." Depending on the circumstances, we may or may not charge third parties for this Aggregate Information. We also may not limit the third parties’ use of the Aggregate Information, except that we do require third parties to whom we disclose Aggregate Information to agree that they will not attempt to make this information personally identifiable by combining it with other databases or otherwise.

How MyHealthReach Workbook Handles Privacy and Security Internally
MyHealthReach wants your Personal Information to remain as secure as possible. That is why we have a two-tier security system at MyHealthReach Online that combines leading technical safeguards and a code of conduct for those employees that are permitted to access our customers' Personal Information. On the technical side, MyHealthReach Workbook uses Secure Socket Layer (SSL) encryption technology in transmitting your personal information to our servers to help ensure the integrity and privacy of the Personal Information you provide to us via the Internet. Encryption involves systematically scrambling numbers and letters, so that even if someone managed to intercept the information, they would not be able to make sense of it. In order to take advantage of this encryption technology, you need to have an Internet browser that will support 128-bit encryption, such as Netscape or Microsoft Explorer. On the employee side, only a limited number of MyHealthReach Workbook employees are authorized to access your Personal Information. All MyHealthReach Workbook employees must abide by our Privacy Policy and those who violate our Privacy Policy are subject to disciplinary action, up to and including termination. Access by authorized personnel is controlled by two-factor authentication (i.e., a token and a thumbprint). Although we will make every reasonable effort to protect your Personal Information from loss, misuse, or alteration by third parties, you should be aware that there is always some risk involved in transmitting information over the Internet. We have strived to make MyHealthReach Workbook safe and secure; however, there is a risk that our system security can be compromised by unauthorized persons or entities.

4. Personal Privacy Options

When you complete Registration or update your Member Profile, you will be given the option of receiving recurring informational/promotional emails from MyHealthReach Workbook and/or third parties. You may opt out of receiving emails from or on behalf of MyHealthReach Workbook at Registration by removing the automatic check mark from the statement that expresses a preference for receiving these emails and newsletters. You may also opt out of receiving these emails and newsletters at any time by changing your preferences within your Member Profile.

We ask for contact information (such as name and email address) if you wish to subscribe to various newsletters. You can unsubscribe from such newsletters very easily. When you have received a newsletter you wish to stop, click on the "reply" button in your mail program, then type in the word "UNSUBSCRIBE" in the "Subject" field and send. MyHealthReach Workbook Customer Service will unsubscribe you from that newsletter in two to three business days.

At Registration and at various times as you use the site, you may have the option of receiving emails or newsletters from third parties, participating in research or marketing surveys and participating in other activities. You may exercise these options by placing a check mark beside a statement that expresses a preference for receiving these communications or participating in these activities. You may change your decision at any time by changing your preferences within your Member Profile or by following other directions provided in association with the individual communication or activity.

This privacy statement applies only to the MyHealthReach Workbook website. The MyHealthReach Workbook site may contain links to other sites. Once you enter another website (whether through an advertisement, service, or content link), be aware that MyHealthReach Online is not responsible for the privacy practices of such other sites. We encourage you to look for and review the privacy statements of each and every web site that you visit through a link or advertisement on MyHealthReach Online's site or any site that collects Personal Information from you.
